Questions and objectives assessments can optionally allow a rating input from the user. To use a rating-style question on a review form, select the appropriate question type when editing the form.
The default rating looks like this, but it can be changed easily using in Configuration > Forms > Rating scale:

In addition to specifying a default custom rating scale for all questions, you can override this for individual questions. For example, you might like to have some Yes or No questions instead. To change the rating options for a specific question, do so on the form editor. Once you've made your changes, they'll override the default rating scale as set in Configuration.
Select Show advanced editing options and then you can either edit the JSON (1) or click Edit next to the scale and re-order/re-name the buttons (2) which will automatically update the JSON.

Click on each button to change the text on the button and the description that appears when you hover over it. You can change the numbers to text if you'd prefer. You can update the colour by specifying a hex code or selecting a colour from the picker to the left of the hex code box. Drag the buttons around to re-order them. Use the trashcan icon to delete or click 'add a button' to add more buttons. Once you've finished, make sure you click 'done editing' otherwise your changes won't save.
If you need to use this updated rating scale for further questions, either use the copy tool on the question itself to duplicate (and then change the question text while leaving the rating scale the same on the duplicated questions), or copy/paste the new JSON into the 'Custom rating scale JSON' box underneath other questions.
